1C. Aftercare or Continuing Care Services

Continuity of care assumes that youth will be provided the services they need no matter what system is responsible for their implementation. Placement decisions need to take into account the level of continuing risk a youth places on the community and to themselves as well as the strengths and needs that are identified in support of a successful transition. Based on a comprehensive and integrated assessment of mental health and substance abuse problems, appropriate treatment can be identified with careful consideration given to cultural and community expectations. Which services will be provided depends on the issues identified in both the pre-release plan and community linkages as well as the following considerations:

  • Level of supervision required, e.g., electronic monitoring, urinalysis screening
  • Acuity of mental health and substance disorder
  • Family and community support and resources
  • Educational and vocational options
  • Transportation needs
  • Access to positive peers
